Insurance & Self-Pay Intake
Insurance and Self-Pay Intake: Select this option if you will be using your private health insurance or if you plan to pay for your mental health treatment out of pocket (self-pay).
Life Coaching Intake
Select this option if you are enrolling in Freedom Fighters Therapy Life Coaching services. Life coaching is a non-clinical service designed to help you identify goals, overcome obstacles, build healthy habits, and achieve personal or professional growth. Life coaching is not therapy, counseling, or mental health treatment.
